By accepting:

1. I acknowledge that I have chosen to participate in the upcoming Soltis Client Event (the “Soltis Event”), that travel to and from the Soltis Event may be dangerous, and that Soltis Investment Advisors, LLC. (the “Company”) cannot guarantee my safety. I further acknowledge that I may choose to participate in various activities at the Soltis Event, which activities may be dangerous, and that the Company cannot guarantee my safety.

2. I acknowledge that the Soltis Event may include presentations, discussions, and materials that are for informational and educational purposes only and are not intended to provide investment advice specific to any individual. Any investment- related content reflects general market information and should not be construed as a recommendation or solicitation.

3. I acknowledge that participating in the Soltis Event and the activities at the Soltis Event are voluntary, and that any failure to participate in the Soltis Event will have no negative impact on my relationship with the Company. Participation does not create any obligation to engage the Company for advisory or other services.

4. In consideration of the Company allowing me to participate in the Soltis Event, I, on behalf of myself and my family, my children and my heirs, executors, administrators, agents, representatives and insurers, assume all risks associated with my participation in the Soltis Event and I agree not to sue the Company and to hold harmless and discharge the Company and each of its officers, directors, employees, agents, volunteers and representatives (the “Released Parties”) from any and all claims, demands, causes of action, liabilities, losses, damages, costs and expenses of any nature, including, without limitation, reasonable attorneys’ fees, arising out of or related in any way to my participation in the Soltis Event, including, without limitation, any and all claims for personal injury of any kind, death, accident or property damage or loss. I further agree, on behalf of myself and my heirs, executors, administrators, agents, representatives and insurers, to indemnify and hold harmless the Released Parties from and against any and all claims, demands, causes of action, liabilities, losses, damages, costs and expenses of any nature (including, without limitation, reasonable attorneys’ fees) arising out of any complaint, lawsuit or other legal proceeding which may be instituted against the Released Parties which arises out of or relates to my participation in the Soltis Event. This provision is intended to be enforced to the fullest extent permitted by applicable law and is not intended to waive any rights that cannot lawfully be waived

5. I acknowledge and agree that the Released Parties assume no responsibility for any liability, damage or injury that may be caused by my acts or omissions related to or during my participation in the Soltis Event or for any liability, damage or injury caused by the acts or omissions of any other participant in the Soltis Event or any other person, except to the extent caused by the gross negligence or willful misconduct of the Released Parties.

6. I agree to obey all applicable rules, regulations and laws and any rules issued by the Company in connection with the Soltis Event. I understand I am responsible for consulting my physician regarding my ability to participate.

7. I represent that I am physically capable of participating in the Soltis Event and that I have no known health restrictions that might jeopardize my safety or health or the safety or health of others during the Soltis Event.

8. I agree that if any provision of these Terms and Conditions is determined to be unenforceable by a court of competent jurisdiction, the remaining provisions of these Terms and Conditions will remain intact and enforceable to the fullest extent permitted by law.

9. I agree that any suit filed in reference to the Soltis Event or to interpret these Terms and Conditions will be filed in Washington County, Utah and be governed by Utah Law. These Terms and Conditions shall continue in effect after the Soltis Event has concluded.

10. I agree that these Terms and Conditions are the only, sole, entire and complete agreement of the parties relating in any way to the subject matter of these Terms and Conditions. I agree that no statements, promises or representations have been made by any party to any other, or relied upon, and no consideration has been offered or promised other than as may be expressly provided herein. I agree that these Terms and Conditions supersede any earlier written or oral understandings or agreements between the Company and me except for any written agreements relating to advisory services or other business arrangements between the Company, which shall remain in effect according to their terms.

11. I agree that, should the Company or any of the other Released Parties be forced to defend a claim, suit or other legal action taken by me or on my behalf, I will pay the Company and any of the other Released Parties any costs and attorney’s fees that they may incur if they successfully defend such claim, suit or action, to the extent permitted by law.

12. A copy of these Terms and Conditions, acknowledged by me, shall be treated as an original.

I give my permission for transportation to any medical facility or hospital and authorize any qualified medical provider to give me the medical care judged to be necessary. I understand that the Company is not providing medical services and is not responsible for the quality or outcome of any medical treatment.

I agree NOT to use illegal drugs or excessive alcohol during any part of the activity and acknowledge that impairment may increase the risk of injury and may result in removal from the Soltis Event.

I hereby give consent to the Company to take and use images (photographs or videotape) or sounds recordings of me in any public media, including radio, television, internet or print, or in Company publications. I understand such images and recordings may be used for marketing or promotional purposes. I may opt out of future use by contacting the Company in writing, to the extent permitted by law.

I of my own free will, for myself and my family, my children and my heirs, executors, administrators, agents, representatives and insurers, have read, understand and acknowledge the risks and liability for myself with respect to the Soltis Event.
